Most people who have heard of TMS associate it with depression, and that is where the majority of its use sits. But the NeuroStar Advanced Therapy System is also intended for use as an adjunct in the treatment of adult patients with obsessive compulsive disorder.
That distinction matters for anyone in Howard County who has worked through the standard OCD treatment path and is looking at what remains.
The established first-line treatments for OCD are exposure and response prevention therapy, a specific form of cognitive behavioral therapy, and SSRI medication, often at higher doses than are used for depression. Many patients respond well to one or both.
Some do not. For those patients, the question becomes what to add rather than what to replace, and this is where an adjunct treatment is relevant. TMS is not positioned as a substitute for therapy or medication. It is used alongside them.
Dr. Greenberger’s evaluation is where that determination gets made. It covers your OCD history, what you have already tried, how you responded, and whether adding TMS is clinically reasonable in your case.

Evaluation with Dr. Greenberger. A 60 minute appointment covering your mental health and psychiatric history in detail, including your OCD treatment history.
Motor threshold determination. On the first treatment day, Dr. Greenberger works with a certified TMS Treater to identify your individual treatment location and determine your dose. Anatomy varies between patients, so this is personalized. The visit runs about 60 to 90 minutes and includes the first treatment.
Treatment course. Your coordinates are recorded so subsequent sessions begin without repeating the setup. You remain awake and alert during treatment and return to normal activity the same day.
OCD is a difficult condition to treat and any practice that tells you otherwise is not being straight with you. Response to any OCD treatment varies considerably between individuals, and TMS is no exception.
What we can say plainly: the treatment is FDA cleared for this use as an adjunct, it is non-invasive, it does not require medication, and the evaluation costs you nothing. Whether it makes sense for you is a clinical judgment Dr. Greenberger will make with you, not a decision made in advance.
The most common side effect of TMS is pain or discomfort at or near the treatment site. It occurs during the treatment course and for most patients does not continue past the first week. There is a rare risk of seizure associated with TMS therapy, occurring in fewer than 0.1 percent of patients.
TMS does not involve anesthesia or sedation, and it does not carry the systemic side effects associated with oral medication.
We accept all health insurances. Our staff works directly with your insurance company to obtain prior authorization, which is typically required for TMS. The adolescent indication for NeuroStar covers major depressive disorder in patients ages 15 to 21. The OCD indication is for adult patients. Dr. Greenberger can discuss what is appropriate for a younger patient during evaluation.
The treatment protocol and targeting differ. Dr. Greenberger determines the appropriate approach during the evaluation and motor threshold determination.
