Anxiety and depression frequently occur in the same patient, and when they do, treatment tends to be harder. Patients with anxious depression often respond less completely to antidepressant medication and experience more relapses than patients with depression alone.
If you have been treated for depression and the anxiety has persisted, or if the two have always been tangled together, you are describing a common and clinically recognized presentation rather than an unusual one.
The NeuroStar Advanced Therapy System is indicated for the treatment of depressive episodes and for decreasing anxiety symptoms in patients who may exhibit comorbid anxiety symptoms, in adult patients with major depressive disorder who failed to achieve satisfactory improvement from previous antidepressant medication in the current episode.
That wording is worth reading closely, because it is specific. The indication covers anxiety symptoms occurring alongside major depressive disorder. It is not a clearance for generalized anxiety disorder on its own. Dr. Greenberger’s evaluation determines where your presentation falls.
A patient who arrives saying “I am anxious all the time” and a patient who arrives saying “I am depressed and also anxious” may look similar in a fifteen minute appointment and require different treatment.
The evaluation with Dr. Greenberger runs a full 60 minutes and covers your psychiatric history in detail, precisely because getting this right determines whether TMS is appropriate. Twenty years of clinical practice across both adult and adolescent psychiatry is what that assessment draws on.
In a large registry study of NeuroStar TMS patients, 83 percent of those who completed the treatment course showed measurable improvement in depression symptoms, and 62 percent no longer met diagnostic criteria for major depressive disorder afterward (Sackeim et al., 2020, Journal of Affective Disorders).

Motor threshold determination. On your first treatment day, Dr. Greenberger and a certified TMS Treater identify your individual treatment location and dose. This visit runs about 60 to 90 minutes and includes your first treatment.
Treatment course. Coordinates are recorded so later sessions start without repeating setup. Patients remain awake and alert and return to normal activity the same day.
The most common side effect is pain or discomfort at or near the treatment site, which is temporary and for most patients stops after the first week of treatment. There is a rare risk of seizure associated with TMS, occurring in fewer than 0.1 percent of patients.
For patients whose anxiety has been worsened by medication side effects, the absence of systemic drug effects is often the reason they consider TMS at all.
